• Students can often be seen right away; if not, an appointment can be scheduled

• Complete our Health Center Referral Form or use our QR Code to schedule an
  appointment

Important Info

• The Health Center is operated by the City of Berkeley
• All services are free for students (in some instances, Alameda County Medi-Cal may be billed for services provided)
• First Aid services require parent/caregiver consent
• Minors 12 and older can consent to their own Mental Health and Reproductive & Sexual Health services and these services are confidential
• All Health Center staff are mandated reporters and confidentiality has some important exceptions associated with mandated reporting
• If Health Center staff need to collaborate with others in order to coordinate care, staff will ask students to complete a Release of Information (ROI) form in order to give staff legal permission to share the minimum necessary Protected Health Information (PHI) with relevant people/programs
• Services are provided in English and Spanish; staff use a Language Line for translation with all other languages as needed
